File Name	Description / Comment
Financial Policy	This file must be kept on record for all clients. You only need to sign it once unless it is updated.
New Client	This file is for clients who have never had an appointment or have not been to our clinic in 3 years or more.
New Patient (Pet) - Current Client	This file is for current clients that have acquired a new pet in the family. New clients may also use this form if bringing in more than one pet to the first visit.
